How to submit

- 1. Complete the pre-application — it checks off the list above and pre-fills the rest of your packet.
- 2. Download all PDFs: this checklist, the pre-application, the terminal request form and the equipment placement agreement.
- 3. Sign the equipment placement agreement where indicated.
- 4. Email every downloaded form plus the documents checked above to support@paydivy.com.
